These are not defects, nor are they shortcomings or any item that could put the project in any sort of jeopardy. These are items that may or may not be directly linked with the project but may have an impact on the project's scope, timeline, deliverables, resources, milestones, budget and operations. Use this form to track any issue during a project's lifecycle.

"Nice document, except ... The documents states that issues "may" impact project activities. Isn't that what risks are? Issues have an immediate impect, so "may" is mis-leading. Just my thoughts."
