Format: Checklist / Sign-off Form

"I think the Configuration Management Form lacks some important details. Whenever the development team hands over new system components that should provide at least a brief description of the changes made and some suggestions for testing the components. If a developer repairs a system bug, the form should describe the bug, how to reproduce the bug, and a simple description of the resolution. The developer could attached the Standard Bug report to eliminate redundant paper work."
