This template rates vendors across ten dimensions. Each dimension has a weight associated to it that denotes the importance or impact on the total rating. Additionally, each dimension has a number of subcomponents that can be used in the evaluation effort. For each dimension rate the vendor’s performance on a scale of 1 to 10 (1 being low and 10 being high). Then multiply the weight and score together for that dimension to arrive at a weighted score. When all the dimensions have been performance rated, sum the weighted score column to get an overall score for the vendor (100 percent would be perfect).
